Loftus Jr., MD<\/b>, discusses lingering questions on the potential risks that gender-affirming hormone therapy may have for transgender patients with inflammatory bowel disease. <\/p>\n<p>\u201cThere are a lot of unanswered questions in this field,\u201d Loftus, the Maxine and Jack Zarrow Family Professor of Gastroenterology at Mayo Clinic and chief medical editor of <i>Healio Gastroenterology<\/i>, said. \u201cWe have some data that gender-affirming hormone therapy  doesn\u2019t appear to increase the risk of <a rel=\"noopener noreferrer\" href=\"https:\/\/www.healio.com\/news\/gastroenterology\/20250226\/qa-wearable-devices-may-help-ibd-patients-track-flares-in-the-comfort-of-their-homes\" id=\"rId10\" target=\"_blank\">IBD flares<\/a> in transgender individuals.\u201d <\/p>\n<p>In their 2023 retrospective multicenter study published in <i>Clinical Gastroenterology and Hepatology<\/i><i>, <\/i>researchers <b>Victor Chedid, MD, MS,<\/b> and <b>Kira Newman, MD, PhD<\/b><b>, <\/b>found no overall increase in IBD flares in the first year after patients started hormone therapy.<\/p>\n<p>However, their findings did highlight one crucial caveat: Patients with IBD who began hormone therapy with active disease or ongoing inflammation were more likely to experience a flare in the year that followed.<\/p>\n<p>Clinicians should also consider the risk for <a rel=\"noopener noreferrer\" href=\"https:\/\/www.healio.com\/news\/gastroenterology\/20250911\/patients-with-ibd-have-heightened-risk-for-blood-clots-after-colorectal-cancer-surgery\" id=\"rId11\" target=\"_blank\">venous thromboembolism<\/a> in patients with IBD receiving hormone therapy. Oral estradiol is known to increase VTE risk, which may be particularly relevant for patients with IBD, who are already at two- to threefold greater risk for VTE than the general population. One \u201cunanswered question\u201d is how overlapping risk factors interact, particularly when hospitalization adds another layer of thrombosis risk to the mix.<\/p>\n<p>\u201cThere is the question remaining about thromboembolism risk that needs to be further addressed,\u201d Loftus said. \u201cThere might be a role for temporarily stopping such therapy in patients with severe inflammation or hospitalization.\u201d<\/p>\n<p>In this <a rel=\"noopener noreferrer\" href=\"https:\/\/www.healio.com\/news\/gastroenterology\/20260604\/urgent-need-for-more-research-nuance-on-ibd-for-patients-who-are-transgender\" id=\"rId12\" target=\"_blank\">month\u2019s issue of <i>Healio Gastroenterology<\/i><\/a>, Chedid, Newman and other experts from Rainbows in Gastro explore the evolving challenges facing transgender patients with IBD as access to gender-affirming hormone therapy comes under increasing political scrutiny.
