Rothenberg<\/b><b>,<\/b><b> MD, PhD,<\/b> director of the Cincinnati Center for Eosinophilic Disorders, told Healio. \u201cA major motivation for the trial was the growing body of evidence showing that EoG is a type 2 inflammatory disease.\u201d<\/p>\n<p>Prior studies have shown increased interleukin-4- and IL-13-associated immune responses in patients with EoG.<\/p>\n<p>\u201cSince dupilumab blocks signaling through the IL-4 and IL-13 pathways and had already <a rel=\"noopener noreferrer\" href=\"https:\/\/www.healio.com\/news\/gastroenterology\/20260519\/dupilumab-boosts-esophageal-distensibility-diameter-for-eoe-in-first-time-showing\" id=\"rId10\" target=\"_blank\">demonstrated efficacy<\/a> in eosinophilic esophagitis, we hypothesized that it could also be effective in eosinophilic gastritis,\u201d Rothenberg said.<\/p>\n<p>Rothenberg and colleagues conducted a proof-of-concept, multicenter, randomized controlled trial to investigate.<\/p>\n<p>They randomly assigned 41 patients aged 12 to 59 years (mean age, 30.5 years; 61% female; 90% white) with histologically active EoG to placebo (n = 20) or a one-time 600 mg dose of dupilumab (Dupixent; Regeneron, Sanofi) followed by 300 mg biweekly (n = 21) for a total of six injections over 12 weeks.<\/p>\n<p>Thirty-seven patients then participated in an unblinded, 24-week, open-label extension period.<\/p>\n<p>Relative change from baseline in mean gastric eosinophil count, an indicator of inflammation, served as the primary endpoint. Secondary endpoints included changes in endoscopic and histologic severity scores.<\/p>\n<p>Patients treated with dupilumab experienced an estimated 50% reduction (95% CI, \u201366 to \u201334) in mean gastric eosinophil count at week 12 compared with a 4% drop (95% CI, \u201320 to 13) in the placebo group.<\/p>\n<p>Absolute changes from baseline also were greater in the dupilumab group for EoG Histologic Scoring System (EoG-HSS) total score (\u20130.12 vs. \u20130.01 points), mean gastric eosinophil count (\u201336 vs. \u20133.2 eosinophils\/HPF) and EoG Endoscopic Reference System (EoG-REFS) total score (\u20133.47 vs. \u20130.06 points).<\/p>\n<p>\u201cTaken together, these findings provide strong evidence that targeting the IL-4\/IL-13 pathway can effectively improve objective measures of eosinophilic gastritis and support the concept that EoG shares key disease mechanisms with <a rel=\"noopener noreferrer\" href=\"https:\/\/www.healio.com\/news\/gastroenterology\/20250730\/esophageal-dilation-may-affect-symptom-response-to-medical-therapy-in-eoe\" id=\"rId11\" target=\"_blank\">eosinophilic esophagitis<\/a> and other type 2 inflammatory disorders,\u201d Rothenberg said.<\/p>\n<p>Incidence of adverse events at week 12 was similar between dupilumab and placebo groups (81% vs. 85%), with blood eosinophilia being the most common treatment-emergent event (29% vs. 30%).<\/p>\n<p>\u201cDupilumab was generally well tolerated in this trial, with no serious adverse events or treatment-related deaths reported,\u201d Rothenberg added.<\/p>\n<p>Researchers did not include symptom improvement as a coprimary outcome, as no validated tool to assess EoG symptoms exists. However, participants\u2019 EoG Symptom Questionnaire eDiary scores revealed no significant change from baseline in either group.<\/p>\n<p>Improvements in mean gastric eosinophil count and EoG-HSS and EoG-REFS scores were sustained through 36 weeks among participants who continued in the open-label extension portion of the trial.<\/p>\n<p>Rothenberg and colleagues postulate that more frequent dosing over a longer period might improve EoG symptoms.<\/p>\n<p>A crucial limitation of this trial is its small sample size.
